    Direct-acting antivirals and hepatocellular carcinoma in chronic hepatitis C: A few lights and many shadows

    With the introduction of direct-acting antiviral agents (DAA), the rate of sustained virological response (SVR) in the treatment of hepatitis C virus (HCV) has radically improved to over 95%. Robust scientific evidence supports a beneficial role of SVR after interferon therapy in the progression of cirrhosis, resulting in a decreased incidence of hepatocellular carcinoma (HCC). However, a debate on the impact of DAAs on the development of HCC is ongoing. This review aimed to analyse the scientific literature regarding the risk of HCC in terms of its recurrence and occurrence after the use of DAAs to eradicate HCV infection. Among 11 studies examining HCC occurrence, the de novo incidence rate ranged from 0 to 7.4% (maximum follow-up: 18 mo). Among 18 studies regarding HCC recurrence, the rate ranged from 0 to 54.4% (maximum "not well-defined" followup: 32 mo). This review highlights the major difficulties in interpreting data and reconciling the results of the included studies. These difficulties include heterogeneous cohorts, potential misclassifications of HCC prior to DAA therapy, the absence of an adequate control group, short follow-up times and different kinds of follow-up. Moreover, no clinical feature-based scoring system accounts for the molecular characteristics and pathobiology of the tumours. Nonetheless, this review does not suggest that there is a higher rate of de novo HCC occurrence or recurrence after DAA therapy in patients with previous HCV infection. \ua9 2018 The Author(s).     Rates of adverse events in patients with ulcerative colitis undergoing colectomy during treatment with tofacitinib vs biologics : a multicenter observational study

    Three hundred one patients (64 tofacitinib, 162 anti-tumor necrosis factor-a agents, 54 vedolizumab, and 21 ustekinumab) were included. No significant differences were reported in any outcome, except for a higher rate of early VTE with anti-tumor necrosis factor-a agents (P 5 0.047) and of late VTE with vedolizumab (P 5 0.03). In the multivariate analysis, drug class was not associated with a higher risk of any early and late complications. Urgent colectomy increased the risk of any early (odds ratio [OR] 1.92, 95% confidence interval [CI] 1.06–3.48) complications, early hospital readmission (OR 4.79, 95% CI 1.12–20.58), and early redo surgery (OR 7.49, 95% CI 1.17–47.85). A high steroid dose increased the risk of any early complications (OR 1.96, 95% CI 1.08–3.57), early surgical site complications (OR 2.03, 95% CI 1.01–4.09), and early redo surgery (OR 7.52, 95% CI 1.42–39.82). Laparoscopic surgery decreased the risk of any early complications (OR 0.54, 95% CI 0.29–1.00), early infections (OR 0.39, 95% CI 0.18–0.85), and late hospital readmissions (OR 0.34, 95% CI 0.12–1.00).peer-reviewe

    Energy Resolution Performance of the CMS Electromagnetic Calorimeter

    The energy resolution performance of the CMS lead tungstate crystal electromagnetic calorimeter is presented. Measurements were made with an electron beam using a fully equipped supermodule of the calorimeter barrel. Results are given both for electrons incident on the centre of crystals and for electrons distributed uniformly over the calorimeter surface. The electron energy is reconstructed in matrices of 3 times 3 or 5 times 5 crystals centred on the crystal containing the maximum energy. Corrections for variations in the shower containment are applied in the case of uniform incidence. The resolution measured is consistent with the design goals

    Verso un inquadramento del rapporto tra informazione di bilancio e modelli di governo delle imprese

    Un’analisi accurata delle determinanti della performance economica e finanziaria delle imprese non può evidentemente prescindere dai caratteri del suo modello di governance. Tuttavia, gli attuali contributi teorici proposti dalla letteratura non indicano in maniera univoca se la relazione che lega le strutture di governo alla performance d’impresa sia, o meno, influenzata dalla qualità della comunicazione economica e finanziaria. In particolare si vorrebbe comprendere se assetti di governo e qualità dell’accounting rappresentino meccanismi sostituti o complementi che le aziende utilizzano per conseguire livelli di performance elevati. Alcuni studi condotti nelle maggiori riviste internazionali di accounting evidenziano chiare tendenze del management ad intervenire sulle stime reddituali e sulla rappresentazione della situazione economico-patrimoniale per raggiungere obiettivi specifici ed orientare la percezione di rischio dell’investimento. Nel nostro Paese, rispetto al quale il modello della corporation si presenta marginale e non riflette l’assetto proprietario prevalente, l’indagine può risultare particolarmente interessante, anche tenendo conto dei limiti applicativi di approcci teorici altrove soverchianti (p.e. teoria dell’agenzia) e contributiva per una migliore comprensione del nostro sistema capitalistico. I risultati attesi dall’analisi potrebbero (o meno) confermare l’idea che la comunicazione d’impresa e le strutture di governo rappresentino meccanismi che influenzano in maniera complementare la performance aziendale e che dunque un modello di governo generalmente considerato “di qualità”, incentivando la bontà della comunicazione finanziaria, produce conseguenze economiche positive per l’impresa
